Animal Abuse Registry
You may only subscribe to the New York City Animal Abuse Registry if you are a NYC animal control officer, member of an animal rescue group that adopts out animals, a veterinarian, or employee of an animal shelter, humane society or pet shop. Employees and members of these business and organizations are required by the New York City Animal Abuse Registration Act (Local Law 4 of 2014) to consult this Animal Abuse Registry before selling or transferring an animal. Transfer or sale of an animal to individuals listed on the Registry is prohibited. Organizations may be verified to confirm eligibility. City Health Information (CHI) Subscription
City Health Information (CHI) is a free publication of the New York City Department of Health and Mental Hygiene that offers the City’s primary care providers clinical guidelines on topics of public health significance. The success of our efforts to improve the health of all New Yorkers depends on the public health-medical care partnership, and the CHI has been an integral part of this relationship since it was first published in 1982.
Citywide Immunization Registry (CIR)
The Citywide Immunization Registry (CIR) is a computerized system that, since 1997, keeps immunization records of people vaccinated in New York City (NYC). The CIR can help ensure that NYC residents receive all required immunizations and are protected from vaccine-preventable diseases. The CIR makes available to agencies and individuals concerned with health (e.g., health care providers, health plans, schools, parents, legal guardians or custodians) immunization records on individuals and, in partnership with the Lead Poisoning Prevention Program, blood lead test information on pediatric patients.
If you are a health care provider, school, or agency concerned with health, and if you are interested in CIR access and reporting, and/or intend to submit immunization data to fulfill Meaningful Use (MU) requirements, you may register here to obtain a facility code.

Group Child Care and Summer Camp Orientation Sessions
The Bureau of Child Care (BCC) provides orientation sessions for individuals who intend to operate a group child care service (non-residential) and for new camp and aquatics directors employed in a summer camp.

Health Academy Training Courses
Health Academy provides training and certification courses for individuals from the public as mandated by the New York City Health Code and trains Environmental Health Personnel in the principles of Environmental Sciences.

NYC Child Care Connect
The Bureau of Child Care (BCC) licenses, regulates and inspects child care services, and assists the public in making informed decisions regarding child care in New York City. The NYC Child Care Connect Service provides regular email and/or text updates on important topics about child care centers, summer camps and school-based child care. You may also sign up to receive notices about specific child care services you have selected.

Public Health Emergency Response Network Auxiliary Distribution Program (PHERN ADP)
The PHERN ADP is a program developed by NYC DOHMH to assist in medical countermeasure distribution to the public in the event of a widespread outbreak of disease. The PHERN ADP targets those who would have difficulty attending public Points of Distribution (PODs) to obtain medical countermeasures – notably residents of congregate care facilities.

Public Health Emergency Response Network Pharmacy Program
The Public Health Emergency Response Network Pharmacy Program (PHERN PP) is an easy-to-use application that allows pharmacies in NYC to provide DOHMH pharmacy contact and service information. This information will assist the City in engaging pharmacies in a productive exchange of information during emergencies to ensure the public's access to critical medications.
Water Tank Inspection Reporting
NYC Health Code requires that drinking water storage tanks be inspected annually and that the results be reported to the Health Department. Register and report your water tank’s inspection results here.
